Onsite Full Time Healthcare

Presbyterian Medical Group is seeking a BE/BC Family Medicine or Internal Medicine physician to join their Medicare Advantage Clinic in New Mexico. This full-time opportunity focuses on providing patient-centric primary care services, emphasizing access to care, value-aligned prescribing, and chronic disease management. The physician will work collaboratively within a multidisciplinary team to deliver comprehensive care to a small patient panel.

Practice Info

Outpatient, primary care services
Focus on patient centric measures such as access to care, value aligned prescribing, diabetes care, cardiovascular care
Work collaboratively with a team, including nurses, medical assistants, case and care managers, behavioral health clinicians, pharmacist clinicians, and clerical staff
Panel size will be kept small to ensure good access, reduced costs and reduced need for the Emergency Department
Provide comprehensive medical care to patients with acute and chronic illnesses through in-person, telehealth, and asynchronous encounters
14-16 patients per day

Responsibilities

Provide comprehensive medical care to patients with acute and chronic illnesses with a focus on access, cost management, and reduced ED and hospital utilization
Make professional medical judgments in the care of patients seen
Order, conduct, and interpret appropriate diagnostic x-ray and lab tests
Prescribe treatments, medications, and non-pharmacologic therapies with a strong alignment toward value
Document all patient encounters and communications appropriately
Manage the provider/patient relationship
Involved in education and counseling of individuals, families, and groups
Work autonomously as well as in collaboration with other health care providers and staff as needed
