Family Medicine / Internal Medicine Physician
Presbyterian Medical Group is seeking a BE/BC Family Medicine or Internal Medicine physician to join their Medicare Advantage Clinic in New Mexico. This full-time opportunity focuses on providing patient-centric primary care services, emphasizing access to care, value-aligned prescribing, and chronic disease management. The physician will work collaboratively within a multidisciplinary team to deliver comprehensive care to a small patient panel.
Practice Info
- Outpatient, primary care services
- Focus on patient centric measures such as access to care, value aligned prescribing, diabetes care, cardiovascular care
- Work collaboratively with a team, including nurses, medical assistants, case and care managers, behavioral health clinicians, pharmacist clinicians, and clerical staff
- Panel size will be kept small to ensure good access, reduced costs and reduced need for the Emergency Department
- Provide comprehensive medical care to patients with acute and chronic illnesses through in-person, telehealth, and asynchronous encounters
- 14-16 patients per day
Responsibilities
- Provide comprehensive medical care to patients with acute and chronic illnesses with a focus on access, cost management, and reduced ED and hospital utilization
- Make professional medical judgments in the care of patients seen
- Order, conduct, and interpret appropriate diagnostic x-ray and lab tests
- Prescribe treatments, medications, and non-pharmacologic therapies with a strong alignment toward value
- Document all patient encounters and communications appropriately
- Manage the provider/patient relationship
- Involved in education and counseling of individuals, families, and groups
- Work autonomously as well as in collaboration with other health care providers and staff as needed
