Job Description:
HarmonyCares is a leading national value-based provider of in-home primary care services for people with complex healthcare needs. Headquartered out of Troy, Michigan, HarmonyCares operates home-based primary care practices in 14 states. HarmonyCares employs more than 200+ primary care providers to deliver patient-centered care under an integrated, team-based, physician-driven model.
This is a part-time, day position, no weekends or evenings. Chronic Condition Management of a geriatric panel as the PCP 3-days per week (Monday, Tuesday, & Friday). Value-based care model, delivering primary care to patients in Kent, Federal Way, Auburn and Renton areas.
The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications:
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Geriatric training/experience preferred
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver’s license and good driving record
