VitalCaring seeks a compassionate Speech Language Pathologist (SLP) to provide home-based evaluation and treatment for adult patients across our home health and hospice programs. You will assess speech, language, cognitive-communication, and swallowing needs; create evidence-based care plans; and educate patients and caregivers in the home setting. Collaborating with an interdisciplinary team, you’ll document in our EMR, participate in case conferences, and support quality initiatives.
