Shift: 4 X 8hr Days (08:00 - 16:30)
Assignment Details
Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks).
Requirements
- Requires state license
- 1 year of skilled home health experience required
- Certification: BCLS
- OASIS experience required
- Must have own transportation
Responsibilities
- Provide assessment and treatment for speech-related disorders in patients' homes.
- Develop individualized care plans based on patient needs.
- Document patient progress and treatment outcomes effectively.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Educate patients and families on speech disorders and strategies for improvement.
