Description
The Clinical Pharmacy Specialist serves as the medication expert within an interdisciplinary care team, providing direct patient care while leading clinical initiatives that improve medication safety, evidence-based practice, and quality outcomes. This role combines advanced clinical expertise with leadership, education, and program development to support patients with complex specialty disease states.
Responsibilities
- Deliver direct patient-centered pharmaceutical care within specialty practice areas.
- Collaborate with physicians, nurses, and interdisciplinary teams to optimize medication therapy.
- Lead quality improvement initiatives and support evidence-based clinical practice.
- Develop clinical guidelines, policies, and medication-use protocols.
- Provide education to patients, pharmacists, providers, and healthcare staff.
- Support pharmacy leadership through operational improvements and program development.
Requirements
- Graduate of an ACPE-accredited College of Pharmacy; PharmD preferred.
- Current Minnesota Pharmacist license (or eligible within three months of hire).
- Specialty Pharmacy Residency or equivalent health-system experience required.
- Strong knowledge of specialty medication therapy management and clinical pharmacy practice.
- Excellent communication, collaboration, and leadership skills.
- Board certification in a pharmacy specialty preferred.
