Ambassador Personnel is seeking an experienced RN Care Coordinator to support population health initiatives and improve healthcare outcomes across Alabama.

Required Qualifications

• Active Alabama or Compact State RN License
• ADN or higher
• 5+ years of healthcare experience
• Experience in care coordination, case management, population health, patient navigation, referral management, or similar programs
• EHR and healthcare technology experience

Important: This role requires care coordination or population health experience beyond traditional bedside nursing.

Preferred Experience

• FQHC, Rural Health Clinic, or primary care
• Chronic disease management and care gap closure
• Patient outreach and referral management
• Value-based care and quality improvement
