Valley Vista Nursing & Transitional Care is seeking a compassionate, dependable, and motivated Registered Nurse to join our night shift. This opportunity is ideal for an RN with strong clinical judgment, communication skills, and a commitment to providing quality resident care.

Responsibilities

* Assess residents, monitor their conditions, and promptly respond to changes in condition.
* Administer medications and treatments according to physician orders and nursing standards.
* Develop, implement, and evaluate individualized nursing care plans.
* Notify healthcare providers of significant findings and follow through on orders.
* Provide direction and support to LVNs and CNAs to ensure resident care is completed safely and promptly.
* Assist with admissions, transfers, and discharges as assigned.
* Maintain accurate, timely documentation, including assessments, nursing notes, and incident reports.
* Respond to emergencies and coordinate appropriate interventions.
* Complete thorough shift handoffs and communicate relevant updates to the care team.
* Follow infection prevention, medication safety, and fall prevention procedures.
* Communicate respectfully with residents and families while protecting confidentiality.
* Participate in required training, competency evaluations, and quality improvement activities.
* Uphold residents’ rights, dignity, and individual preferences.

Required Qualifications

* Active California RN license in good standing.
* Current BLS/CPR certification.
* Strong assessment, clinical judgment, and organizational skills.
* Ability to prioritize responsibilities and respond effectively to urgent situations.
* Effective leadership, communication, and teamwork skills.
* Dependable attendance and punctuality.
* Ability to perform essential job duties, with or without reasonable accommodation.
* Availability to work 11:00 PM 7:00 AM on a rotating schedule of 4 days on and 2 days off, including weekends and holidays as scheduled.

Preferred Qualifications

* Previous experience in skilled nursing, rehabilitation, or long-term care.
* Experience supervising nursing staff.
* Familiarity with PointClickCare (PCC).
* Experience with wound care, admissions, and changes in resident condition.
