Role: Registered Nurse (RN) providing one-on-one intermittent homecare visits to patients.
Responsibilities
- Utilize the nursing process to plan, implement, and evaluate patient care based on individual needs.
- Develop and revise care plans in collaboration with prescribers as patient conditions change.
- Administer medications and treatments, monitor their effectiveness, and document outcomes.
- Respond to changes in patient condition by intervening appropriately and reporting to physicians or supervisors.
- Educate patients and support care coordination, discharge planning, and goal setting.
- Ensure compliance with professional standards, legal requirements, and organizational policies.
- Participate in team meetings, case conferences, and ongoing education activities.
- Perform additional duties as assigned.
Qualifications & Skills
- Graduate of an accredited nursing program.
- Current RN license for the relevant state.
- Valid BLS certification and current PPD or Chest X-Ray.
- Preferred but not mandatory: one year of RN experience.
- Adherence to professional standards, practice, and legal regulations.
