Role: Registered Nurse (RN) providing one-on-one intermittent homecare visits to patients.

Responsibilities

  • Utilize the nursing process to plan, implement, and evaluate patient care based on individual needs.
  • Develop and revise care plans in collaboration with prescribers as patient conditions change.
  • Administer medications and treatments, monitor their effectiveness, and document outcomes.
  • Respond to changes in patient condition by intervening appropriately and reporting to physicians or supervisors.
  • Educate patients and support care coordination, discharge planning, and goal setting.
  • Ensure compliance with professional standards, legal requirements, and organizational policies.
  • Participate in team meetings, case conferences, and ongoing education activities.
  • Perform additional duties as assigned.

Qualifications & Skills

  • Graduate of an accredited nursing program.
  • Current RN license for the relevant state.
  • Valid BLS certification and current PPD or Chest X-Ray.
  • Preferred but not mandatory: one year of RN experience.
  • Adherence to professional standards, practice, and legal regulations.
