Job Summary And Qualifications
Registered Nurse (RN) Case Manager – support patients and families through every step of their care journey. Coordinate services, connect resources, and develop individualized care plans. Partner with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and consistent quality across the continuum of care.
Responsibilities
- Guide patients and families through program orientation, explain rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
- Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
- Develop and implement individualized treatment plans reflecting each patient’s strengths, needs, and recovery goals.
- Complete psychosocial assessments and discharge planning evaluations, document findings, and communicate needs across the care team.
- Collaborate closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient‑centered care.
- Promote HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction.
Qualifications
- Graduate of an accredited nursing school: Associate’s Degree in Nursing or higher is required; BSN highly preferred.
- Licensed as a Registered Nurse in the state of practice, or hold an active Compact/Multi‑State RN license.
- 1‑3 years of clinical, hospital nursing experience.
- Prior experience in Case Management for the PRN role.
- Certification in Case Management or Utilization Review preferred.
- Experience with InterQual preferred.
