Supports patient flow, communication, and coordination activities to ensure timely, accurate, and high-quality care delivery.

Responsibilities

• Coordinate patient scheduling, messages, records, and follow-up activities to support efficient care delivery.
• Communicate with patients, providers, and staff to ensure accurate information exchange and smooth transitions.
• Maintain documentation, assist with regulatory compliance needs, and support daily operational workflows.
• 1 year as RN Case Manager (Acute/Hospital) - current/recent
• Active RN Oregon & Washington/Compact license
