RN Case Manager - Home Health

Job Title: RN Case Manager – Full‑Time, Memphis, TN.

Beacon Home Health, part of the Pennant Group, Inc., is recruiting an RN Case Manager for its Home Health team.

Position Overview

• Deliver the full plan of care for home‑health patients, continuously evaluating performance to improve productivity and care quality.
• Make assigned patient care visits per physician orders and maintain accurate, timely documentation—including visit reports, OASIS assessments, plans of care, medication profiles, progress notes, recertifications, and discharge/transfer summaries.
• Attend all patient meetings and provide input on progress toward patient goals.
• Understand and adhere to agency policies, procedures, and processes (administrative, personnel, clinical).
• Seek guidance when uncertainty arises.
• Establish and maintain an efficient visit route and provide services within the scope of nursing practice as defined by state law.
• Collaborate daily with patients, families, attending physicians, and interdisciplinary team members on care delivery.

The Must Haves

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and reliable transportation (personal vehicle or public transit) to commute to multiple patient residences daily within the assigned geographic area.
