Position:
RN Care Manager, responsible for assessing patient needs, developing care plans, coordinating services, and facilitating communication among patients, families, and healthcare providers.
- Conducting comprehensive patient assessments of medical, social, and psychological needs.
- Collaborating with healthcare teams to create tailored care plans.
- Identifying barriers to care and coordinating appropriate services.
- Managing patient transitions, referrals, and follow-up appointments.
- Providing education on diagnoses, treatment options, and self-management strategies.
- Maintaining accurate electronic health records and documenting interactions.
- Participating in quality improvement initiatives and ensuring compliance with healthcare regulations.
Key duties include:

Qualifications and skills:
- Bachelor's Degree in Nursing required; master's preferred.
- Minimum of 2 years clinical nursing experience, preferably in acute or hospital settings.
- Current RN license in Illinois.
- Preferred certification as a Case Manager (CCM) or similar.
- CPR/BLS certification for healthcare providers.
- Strong assessment, communication, and collaborative skills.
