RN, Care Manager (Home Health)

Registered Nurse (RN) – Care Manager, Home Health (Contract)

Schedule

8-hour Day Shift (1st Shift)

Monday – Friday

Shift Time: 8:00 AM – 5:00 PM

No routine call or float (may be assigned based on agency needs)

Position Summary & Responsibilities

Manage home health caseload and coordinate patient care plans

Perform assessments, implement care, and monitor patient progress

Collaborate with interdisciplinary teams to meet care goals

Educate patients and families and ensure timely, accurate documentation

Job Requirements

Education & Experience:

BSN: Preferred

Acute care or home health experience: Preferred

Licenses/Certifications (Required):

Active RN license in the state of Oregon

BLS (AHA/HS-BLS/RQI)

Valid Driver’s License (age 21+)

Current auto insurance and reliable vehicle
