- Coordinates and manages the care of patients with chronic or complex conditions
- Works collaboratively with physicians, interdisciplinary teams, individual patients, and families
- Performs comprehensive care continuum process including assessment, planning, implementation, coordination, monitoring, and evaluation
- Advocates for, educates, and coaches patients, families, and caregivers on treatment options
- Supports goal-setting and development of self-care skills
- Facilitates communication and coordination among members of the healthcare team
Requirements
- Current Registered Nurse (RN) license in state of practice
- Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required)
- Demonstrated clinical nursing experience, and familiarity with patient-related terminology and processes
- Demonstrated experience in care management
- Certified Case Manager (CCM) or other relevant certification preferred
- Two (2) years of experience in nursing, preferably in primary care or ambulatory care settings
- One (1) year of experience in care management
- Experience in chronic disease management
- Knowledge of value-based care models and principles
