🔎 Position Overview
As a Registered Nurse (RN), you will provide skilled nursing care to patients in their homes, working under physician orders and care plans, while collaborating with a dedicated clinical team.

- Broomfield & surrounding communities
- Northern Colorado
- Southwest Colorado
✅ Key Responsibilities
- Deliver skilled nursing care based on individualized care plans
- Conduct initial patient assessments and ongoing evaluations
- Coordinate care and communicate changes with physicians and team members
- Educate patients and families on care and health management
- Maintain accurate, timely clinical documentation (including OASIS)
- Participate in care planning, case conferences, and quality improvement efforts
- Supervise LPNs, CNAs, and HHAs per regulatory guidelines
- Support patients’ rehabilitative and preventive care needs
🎓 Qualifications
- Active RN license
- Graduate of an accredited nursing program
- At least 1 year of nursing experience (preferred)
- Strong clinical judgment, communication, and organizational skills
- Current CPR certification
- Ability to work independently and manage time effectively
