They are accountable for patient health, building trust, and acting as a health coach to support behavior change and long-term wellness.

Key Responsibilities

• Independently manage patient care, including assessment, diagnosis, treatment, and care coordination.
• Lead care teams comprising medical assistants, care facilitators, and coordinators.
• Collaborate with hospitalists and other healthcare professionals for patient hospitalizations and transitions.
• Participate in risk and quality management programs, clinical meetings, and adhere to compliance standards.
• Engage in center management, including recruiting patients and providers when needed.
• Supervise and collaborate with Advanced Practice Practitioners, remaining accountable for their actions.

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related specialty; board eligibility or certification preferred.
• Current medical license and DEA registration.
• Basic Life Support (BLS) certification required.
• Strong leadership, communication, and organizational skills.
• Proficiency in evidence‑based medicine, lifestyle and nutritional interventions, and technology tools.
• Availability to travel locally and nationally as needed.
• Commitment to patient-centered care, quality improvement, and team collaboration.
