ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join their rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, acting as a health coach to help patients achieve their health goals.

  • Regularly measured metrics include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures
  • Care team consists of a care promoter (medical assistant), care facilitator, and care coordinator for office patients
  • Engages with transitional care team, case managers, acute and transitional-care physicians, and other resources for patients unable to come to the office

Responsibilities

  • Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
  • Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
  • Engage with the hospitalist when a patient is hospitalized
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
  • Lead the care team
  • Actively manage the center and help cover for other providers
  • Take an active role in recruiting patients for the center and additional providers for the company

Compensation

  • $214,700 - $306,714 Salary

Requirements

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
  • Ability to obtain a State Medical License or already possess a current, active State Medical License for the state(s) of work
  • Board Eligibility is required
  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
  • Maintain board certification in specialty by doing necessary MOC, CME and/or retaking board exams as required
  • Current DEA number for schedule II-V controlled substances
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employment
  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
  • Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
  • Fluency in English, verbal and written
