ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for this population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, working to build trusting relationships and coach patients towards better health.

- PCPs lead a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office patients
- Engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources for homebound patients or those in hospital, SNF, or LTC
- PCPs will have an active role in the management of their center and will help cover for other providers
- PCPs are expected to take an active role in recruiting patients for the center and additional providers for the company
Responsibilities
- Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
- Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
- Engage with the hospitalist whenever one of their patients is in the hospital
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
- Lead their care team for patients able to come to the office
- Adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures
- Participate in risk and quality management programs, clinical meetings, and other required meetings that promote patient health and company goals
- Perform other duties as assigned and modified at manager’s discretion

- $221,141 - $315,915 Salary
- This position may also be eligible for a bonuses or commissions

- Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes
- Fluency in English, verbal and written
