Physical Therapist – Home Health

Day shift, 5 days per week from 08:00 to 16:30.

Assignment Details

Full-time position with approximately 7 visits per day. Full-time productivity is around 25 units.

Requirements

• Minimum of 1 year of experience; first-time travelers are accepted.
• Active BLS certification.
• GA Physical Therapy License or Compact PT License accepted.
• Experience with Home Care Home Base (HCHB) preferred; online course required if not already familiar.
• Home Health experience, particularly with Geriatric, Stroke, and Orthopedic patients.

Responsibilities

• Provide physical therapy services to adult and geriatric populations in a home health setting.
• Evaluate and implement individualized patient care plans.
• Monitor and document patient progress and adjust treatment plans as necessary.
• Collaborate with healthcare team members to support patient outcomes.
• Educate patients and families on therapeutic exercises and overall health management.

Compliance / Notes

Full compliance required to start on time; if not compliant in time, start may be delayed by 2 weeks. If one major holiday falls during the assignment, the traveler is expected to work it. If two major holidays occur, the traveler is expected to work both. If three major holidays exist, the traveler is expected to work two of the holidays.
