Pay: PT: $80 per visit; PTA: $55-$60 per visit. Location: O’ahu, HI (reliable transportation required). Schedule: Flexible / Per Diem (PRN) – Build Your Own Caseload.

Responsibilities
- Provide skilled in‑home physical therapy services.
- Complete evaluations, reassessments, and fall risk assessments (PT).
- Implement individualized treatment plans to improve mobility, balance, strength, and safety.
- Educate patients and caregivers on home exercise programs and fall prevention.
- Document accurately and communicate patient progress with the interdisciplinary care team.
- Maintain compliance with Medicare home health documentation requirements.
Requirements
- Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA).
- Minimum one year of clinical experience (home health experience preferred).
- Strong documentation, communication, and time‑management skills.
- Ability to work independently in the field.
- Valid driver’s license and reliable transportation.
- Passion for serving seniors, kupuna, and local families.
