Salary Estimate: $71,947.20 - $107,931.20 / year

Candidates must live in the HCA HealthONE Presbyterian/ St. Luke's and Rocky Mountain Children's in the Denver, CO area and will focus on Perinatal patients.

Job Summary and Qualifications

As a Nurse Navigator, you will function as a member of the multidisciplinary team and serve as an advocate and educator for patients with all types of medical conditions. You will be responsible for supporting patients through complex healthcare environments for the optimization of care and outcomes. You will facilitate patient follow‑up through coaching, information transfer, and communication to improve continuity of care, enhance the medical management of complex diseases, and streamline the intervention process. You will also guide patients and families through the healthcare system by assisting with access issues to expedite the timeliness of care and treatment. You will achieve this goal by building relationships with patients and physicians, coordinating the defined plan of care, assisting with appointments, resolving barriers, providing education, and connecting with community resources. You will assume responsibility and accountability for the management of resources to achieve efficient, high‑quality outcomes for patients, including support with interdisciplinary and cross‑facility coverage and collaboration. You are responsible for timely data collection that promotes tracking of the navigation program performance in line with HCA Healthcare, division, and facility goals (growth, quality, practice guidelines, etc.).

What you will do in this role

  • Serve as a patient advocate
  • Assess the patient’s medical and psychosocial needs as they pertain to their primary medical or surgical condition for navigation
  • Use appropriate tools to identify patient needs and barriers to care and provide access to potential resolutions
  • Identify health disparities and assist in the removal of these disparities
  • Provide appropriate education, outreach, and support to patients and families; ensure the patient is empowered to manage their healthcare needs
  • Provide healthcare system guidance and promote adherence to recommended treatment throughout the care pathway
  • Support providers by assisting patients and/or families in understanding their diagnosis, treatment options, and the resources available, including education, clinical research studies (as applicable), and technological advances
  • Provide education/connection to community and external resources, such as access to supportive care, financial support, and return to work
  • Streamline process for patients by assisting with appointment scheduling in a compliant manner
  • Ensure the organization of appointments and explain the sequence of treatments
  • Ensure smooth transitions between care modalities, facilities, and providers
  • Facilitate patient movement through the appropriate clinical pathway and collaborate with physicians to ensure patient compliance
  • Reinforce patient‑specific teaching as defined by the patient’s provider
  • Conduct follow‑up conversations with patients and communicate any concerns, changes, or social needs to the appropriate physician or other care provider
  • Attend and participate in Multidisciplinary Meetings (MDM) and/or other care planning conferences as required by the navigation program
  • Ensure appropriate patient data is available, including assessment and documentation
  • Educate the patient and family regarding planned procedures, medications, and treatments

What you will need in this role

  • 1–3 years of experience in Nursing – Required
  • 3+ years of experience as a Nurse Navigator and/or patient educator in service line – Preferred

Education

  • Associate’s Degree in Nursing – required
  • Bachelor’s Degree in Nursing – preferred

Licenses and Certifications

  • Basic Cardiac Life Support – required (preferred if remote position)
  • Registered Nurse – currently licensed as a registered professional nurse in the state in which they practice, in accordance with law and regulation – required
