As a Nurse Practitioner, you will conduct comprehensive assessments in members homes, spending approximately 45 minutes per visit. Assignments may include annual health assessments or post-discharge evaluations in locations such as Topeka, Silver Lake, and Auburn.

Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Gather and evaluate medical history, existing conditions, and medication usage
- Educate patients on their medical conditions and medication regimens
- Identify gaps in care, recommending solutions to enhance patient outcomes
- No treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Kansas
- Board Certification through AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited institution
- Current CPR (BLS) certification
- At least 6 months of recent experience as a Nurse Practitioner preferred
