Home Health Speech Therapist/Language Pathologist
Territory: Solano County: Vallejo, American Canyon, Hercules and minimal visits to Napa County
What you’ll do:
- In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions.
- Evaluate and treat speech, language, cognitive and fluency disorders that affect communication.
- Serve as an educator, patient advocate and member of the interdisciplinary care team.

Qualifications
- Current unrestricted CA Speech Therapist License.
- Current BLS/CPR Certification.
- Valid CA Driver's License.
- Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
- Knowledge of state and federal homecare regulatory guidelines preferred.
- Experience in home health care is highly preferred.
- Experience with high acuity patients is a plus.
- Experience working with an interdisciplinary team is highly desired.
- Case Management fundamentals is highly desired.

Required assessment: Candidates must complete a computer literacy and competency assessment before in-person interview.
