nJob Overview Talin is seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will be responsible for providing high-quality patient care in a patient's homes, focusing on improving the functional abilities of individuals with disabilities, injuries, recovery and aging. This role involves developing personalized care plans, conducting assessments, and implementing therapeutic interventions to enhance patients' quality of life.
nDuties
nConduct comprehensive evaluations to assess patients' physical, emotional, and social needs.
nDevelop and implement individualized care plans tailored to each patient's goals and capabilities.
nProvide direct patient care through therapeutic activities designed to improve daily living skills and overall functionality.
nCollaborate with other healthcare professionals, including speech therapists and case managers, to ensure a holistic approach to patient care.
nEducate patients and their families about treatment options, progress, and strategies for ongoing support.
nMonitor patient progress and adjust treatment plans as necessary based on observed outcomes.
nMaintain accurate documentation of patient assessments, treatment plans, and progress notes in compliance with policies.
nRequirements
nDegree in Occupational Therapy from an accredited program.
nValid state licensure as an Occupational Therapist.
nStrong knowledge of physiology and occupational health principles.
nExperience in case management and working with individuals with disabilities is preferred.
nFamiliarity with early intervention strategies is a plus.
nExcellent communication skills to effectively interact with patients, families, and interdisciplinary teams.
nAbility to develop effective care plans that address individual patient needs.
nCompassionate nature with a strong commitment to patient care and rehabilitation outcomes.
