Job Summary

The Nurse Navigator works closely with physicians in variousspecialty areas and coordinates a patient's plan of care,communication, and program. Position provides expert individualizedcare coordination to patients and their families ensuring a timelyschedule for diagnostic work-up. Position also communicates betweenthe patient and the multidisciplinary team ensuring the patientreceives all the necessary referrals and follow-up.

Major Responsibilities/Duties/Critical Tasks

  • Develop and enhance processes with the aim at improving theclinical experience for patients and their caregivers.
  • Reviews referrals for appropriate testing and information priorto approval by Providers.
  • Coordinate with information technology and quality departmentsto ensure quality measures are met and data is recordedaccurately.
  • Utilize clinical protocols to refer patients for appropriatescreenings and diagnostic testing
  • Maintain knowledge of current state, federal, and facilityrequirements to meet current healthcare goals related to patientsafety, reimbursement, and healthcare quality standards.
  • Work with a multidisciplinary team to develop and implement anup-to-date plan of care.
  • Assess, develop, and implement effective patient and familyteaching to accurately assess readiness to learn.
  • Communicate with all members of the healthcare team on behalfof the patient, caregivers, outside entities that are included inthe care of the patient
  • Assist the patient with referrals to the patient financialcounselor when needed
  • Collaborate with the marketing and outreach departments toeducate referring physicians and local communities about thefacilities and services provided.
  • Establish quality improvement projects with residents andfellows.
  • Maintain a patient database and participate in patientrecruitment for clinical research.
  • Other duties as assigned.

Required Education/Experience

  • Graduate from an accredited school of nursing.
  • Nurse Navigator experience

Required Certifications/Licenses

  • Certified RN. (Must possess and maintain a current license orvalid permit to practice in TX)
  • Possess and maintain current BLS Certification.

  • Strong analytical and problem-solving skills to identify andresolve issues efficiently.
  • Demonstrates excellent communication skills, both verbal andnonverbal.

The retirement plan for this position is Teacher Retirement Systemof Texas (TRS), subject to the position being at least 20 hours perweek and at least 135 days in length.

Clinical Position Requirements

Clinical positions require pre-screening through our OccupationalHealth department, including a drug test. Employees are required toobtain annual influenza vaccines.
