Schedule: Monday-Friday, 8:00 AM - 5:00 PM
Job Summary
We are seeking a compassionate Hospice Medical Social Worker (MSW) to join our care team and provide psychosocial support to hospice patients and their families. The ideal candidate will collaborate with interdisciplinary teams to develop and implement patient-centered care plans while serving patients throughout the Jackson, MI area.
Key Responsibilities
- Assess patients' social, emotional, and financial needs.
- Collaborate with the care team to develop and manage individualized care plans.
- Provide counseling, crisis intervention, and community resource coordination.
- Maintain accurate and timely documentation within EMR systems.
- Travel to patient homes and care facilities within the service area.
- Support patients and families through end-of-life care and transitions.
Required Qualifications
- Master's Degree in Social Work (MSW) from an accredited program.
- Active Michigan MSW license in good standing.
- 1+ years of social work experience in a healthcare setting.
- Experience using EMR/EHR systems (TruChart EHR preferred).
- Valid Driver's License, reliable transportation, and auto insurance.
- Ability to manage schedules independently and meet documentation deadlines.
- Comfortable with local travel.
