Responsibilities
- Lead educational initiatives to improve provider performance, quality of care, and adherence to standard of care practices.
- Conduct monthly discussions with PCPs identified as having clinical management opportunities.
- Conduct monthly meetings to review physician scorecards and KPIs related to ED/admissions, referral management, and patient satisfaction surveys.
- Work with the Territory Medical Director to develop and implement quality improvement projects.
- Oversee and provide leadership to a group of providers within the district.
- Foster a supportive environment for clinical excellence, professional growth, and patient-centered care.
- Conduct regular performance reviews, providing actionable feedback and recognition of achievements.
Requirements
- Must have a Medical Degree from an Accredited Medical School or DO School.
- Board Certification in Family or Internal Medicine.
- Fluent Spanish and English due to staff and patient demographics.
- Experience in Medicare Advantage Risk and Value Based Care Management.
- Experience leading and successfully managing providers.
