Alvita Care is seeking experienced Licensed Practical Nurse (LPN) in Connecticut!

Details:

• We have clients throughout CT (Fairfield, New Haven and Litchfield County) requiring shifts from 6, 8hrs to 12hrs. Day / Night Shifts Available.
• Work with us part time, per diem or around your current work schedule.
• Shifts are assigned to provide 1:1 care in the home environment.

Qualifications:

• CT LPN licensure
• BLS, ACLS, or CPR certification.
• Minimum of One (1) year demonstrated experience as an LPN
• Ability to put patients at ease and explain physician's instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently
• Valid driver's license or NYS Non-Driver photo ID card, required
• Drivers preferred!

• Consult with management on complex/unique cases
• Monitor/Assess patients/clients' condition, effects of the care plan
• Oversee/supervise the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manage, plan, coordinate and execute the patient's safe discharge from service.
• Ensure that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.
• Communicate and consult with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
• Assist patients with activities of daily living
• Document content and process of all services provided in internal system properly.
• Transport/utilize medical equipment to and from clients/patients' home/facilities. Travel to patient/clients' homes and/or other facilities.
• Perform other related duties and participates in special project, as assigned.
