nPosition Summary:
nOmnicare is a leading provider of pharmacy services to long-term care facilities nationwide, supporting skilled nursing facilities, assisted living communities, and other institutional care settings. Following the sale of Omnicare by CVS Health, this role will support Omnicare and its continued operations as an independent, standalone organization. Candidates should be aware that this role will not remain within CVS Health following the transaction. The individual hired into this role will be employed by Omnicare and will contribute to shaping the future of our organization during this exciting period of growth and transformation.
nResponsibilities:
nEnsure compliance with all pharmacy laws and regulations, and sterile compounding guidelines.
nMaintain all required IV documentation and clinical monitoring.
nFully assess new patients and new orders on existing patients for appropriateness of therapy, high risk medications, allergies, adverse indications or interactions, and other potentially adverse outcomes.
nThorough knowledge of prescriber/facility notification process for drug interaction, allergy, and other adverse patient safety situations.
nPerforms accurate dose calculations based on medication, patient condition, volume and administration rates.
nProgram infusion pumps - Consistent and ongoing clinical monitoring of all infusion patients.
nThorough knowledge of medication occurrence reporting process.
nHave a thorough understanding of the Omnicare Long Term Care Pharmacy/Home Infusion Pharmacy Policy and Procedure Manual and internal pharmacy policies and procedures.
nKnowledge of other facility infusion therapy manuals if not using the Omnicare Customer Infusion Procedure manual.
nIV order processing only when technical support unavailable.
nExcellent care coordination with home health agencies, nursing facilities, physicians, and the IV nurse consultant.
nAnswer drug-related questions as related to IV therapy and management in a timely manner.
nCommunicate with facility consultant pharmacists and clinical staff pharmacists regarding IV patient management.
nOrient and provide competency assessment for all pharmacists who rotate from oral pharmacy to IV pharmacy and orient and assess competency on all IV technicians and infusion pump technicians, when appropriate; this includes making certain that all pharmacists and technicians rotating into the IV room have completed the approved Omnicare Infusion Training Program.
nMaintain knowledge of currently available infusion devices, products, and supplies.
nOversee cleanroom monitoring and pump maintenance.
nFollow all applicable government regulations including HIPAA.
nOther duties as assigned; Job duties may vary by location.
nThis is a full time position. Schedule will be based on the needs of the business. Schedule will be Monday-Friday between the hours of 10am-6pm. Weekend rotation is every third. Weekend hours are 10am-6:30pm. There is no on-call requirement.
nEducation:
nBachelor's Degree - Required
nRequired Qualifications:
n1 to 3 years - Required
nBS Degree in Pharmacy or Pharm.D. degree from an accredited School of Pharmacy required
nCurrent license to practice pharmacy in good standing
n3 years infusion therapy experience which MUST include clinical management of patients
nPreferred Qualifications:
n2 years of long term care experience
nImmunization certification or willingness to become immunization certified
nExcellent organizational and time management skills
nExcellent interpersonal skills
nExcellent verbal and written communication skills
nAbility to work independently to plan, develop and coordinate pharmacy activities
nAbility to read, analyze and interpret complex topics using reasoning abilities to apply advanced mathematical concepts and draw valid conclusions.
nPhysical Demands:
nMay sit, stand, stoop, bend and walk intermittently during the day. May sit or stand seven (7) to ten (10) hours per day. May be necessary to work extended hours as needed. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Visual acuity to see and read fine prints. Specific vision abilities required by this role include close vision, color vision and the ability to adjust focus. Must be able to hear normal voice sounds. The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this role.
