In-Home NP/PA Clinician — Personalized Health Evaluations

Signify Health, part of CVS Health, seeks a Full-time Clinician (NP or PA) to deliver in-home health evaluations in the Western Bridgeport area. Each 45-minute visit includes medication review, medical history, and a physical assessment, with potential optional tests ordered by the health plan.

You will work with a national network of clinicians, travel locally as needed, and conduct virtual visits when required.
