Position Summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to provide person-centered care in the home environment. The role involves assessing patient and family needs, delivering professional nursing care based on the full nursing process, and collaborating with the Interdisciplinary Group (IDG) to develop individualized plans of care. This position supports patients and families with empathy, ensuring dignity and comfort during end-of-life care.

Shift and Schedule

Saturday and Sunday, 2x12-hour shifts.

Position Specific Responsibilities

  • Completes assessment of patient and family to determine hospice needs.
  • Provides a complete physical assessment and history and delivers professional nursing care using all elements of the nursing process.
  • Writes and initiates Plan of Care in collaboration with the IDG.
  • Reevaluates patient/family/caregiver needs regularly and revises the Plan of Care as necessary.
  • Manages medication and treatments as prescribed by the IDG-approved Plan of Care.
  • Participates in on-call duties as defined by the on-call procedures.
  • Maintains productivity standards set by organizational guidelines.

Documentation and Information Management

  • Completes, maintains, and submits accurate clinical notes regarding patient condition and care delivered.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Participates in orientation, education, and development of team members.
  • Continues professional growth through continuing education and in-service classes.

Education and Experience

  • Required: Associate or Bachelor’s degree from an accredited School of Nursing.
  • Preferred: 1 year of experience as a registered nurse; experience as a Hospice CNA or LPN will be considered.

Certifications, Licenses, and Registrations

  • Required: Current, unencumbered Registered Nurse license (State of Employment).
  • Required: Within 30 days of hire – national provider BLS certification from the American Heart Association.

Physical Demands and Work Environment

The role requires standing, walking, and manual dexterity, lifting up to 50 pounds, and exposure to bodily fluids and infectious diseases. Ability to use standard medical equipment and observe environmental safety is essential.

Compensation

Base pay range: $40.04–$72.09 per hour. Additional compensation may be available for shift differentials, on-call duties, overtime, premiums, and bonuses.
