Position: Home Health Physical Therapist

Employment Type: Full-time/Part-time

Coverage Area: Northen Virginia; Culpepper, Marshall, The Plains, Hume, Linden, Manassas, Woodbridge, Gainsville

Responsibilities

• Conduct comprehensive evaluations of patients' physical and functional abilities
• Develop personalized treatment plans to enhance mobility, relieve pain, and improve overall physical well‑being
• Provide hands‑on therapy and exercises to patients in their home settings
• Educate patients and their families on therapeutic exercises and techniques
• Collaborate with interdisciplinary healthcare professionals to coordinate patient care
• Document and maintain accurate records of patient progress and treatment outcomes

Qualifications

• Valid license as a Physical Therapist in Virginia.
• Experience in home health or a related field is preferred but not mandatory
• Strong clinical skills and knowledge of evidence-based physical therapy practices
• Excellent communication and interpersonal skills to effectively interact with patients, families, and other healthcare team members
• Self‑motivated, compassionate, and dedicated to providing exceptional patient care
• Ability to work independently and manage time efficiently
• Valid driver's license and reliable transportation
