Home Health Physical Therapist - Flexible Schedule
Pay: $80 per visit
Location: O‘ahu, HI
Schedule: Per Diem (PRN)
What You’ll Do
- Perform physical therapy evaluations and reassessments.
- Develop and implement individualized treatment plans.
- Improve mobility, balance, strength, and safety.
- Provide patient and caregiver education.
- Collaborate with physicians, nurses, and healthcare professionals.
- Complete accurate and timely documentation.
Qualifications
- Hawaii Physical Therapist license.
- Minimum one year of clinical experience.
- Home health experience preferred.
- Reliable transportation.
- Strong organizational and communication skills.
