nAbout the Position
nDivine Care Spaces is looking for dependable, compassionate, and experienced caregivers to join our team at our assisted living residence in Brooklyn Park.
nAs a Home Health Aide/Caregiver, you will provide hands-on support to residents with their daily needs while helping maintain a safe, comfortable, and respectful home environment.
nWe are looking for caregivers who are patient, responsible, attentive, and able to work independently while following each resident's individualized care plan.
nResponsibilities
nAssist residents with activities of daily living, including bathing, grooming, dressing, toileting, and mobility
nAssist residents with transfers, repositioning, and safe movement throughout the home
nProvide medication assistance or administration as directed and permitted by the resident's care plan and nursing staff
nMonitor and document resident condition, behaviors, vital signs, and changes in health status as required
nPromptly report health or safety concerns to the appropriate supervisor or nurse
nPrepare and serve meals and assist residents with eating when needed
nPerform light housekeeping duties, including laundry, changing bed linens, cleaning resident areas, and maintaining a clean living environment
nProvide companionship and engage residents in conversation, activities, walks, games, or other appropriate activities
nAssist residents with appointments, outings, and transportation when assigned
nFollow resident care plans, facility policies, infection-control procedures, and safety protocols
nComplete accurate shift documentation and communicate important information to incoming staff
nRespond appropriately to emergencies and changes in resident condition
nMaintain resident privacy, dignity, and confidentiality at all times
nQualifications
nHigh school diploma or GED preferred
nPrevious professional caregiving experience strongly preferred
nExperience in assisted living, home care, group homes, memory care, hospice, or a similar healthcare setting is preferred
nExperience assisting with medication administration and monitoring vital signs is a plus
nMust be comfortable providing hands-on personal care
nMust be dependable and able to work scheduled 12-hour shifts
nAbility to communicate professionally with residents, families, coworkers, nurses, and supervisors
nAbility to complete required documentation accurately
nValid driver's license preferred/required for positions involving resident transportation
nCPR/First Aid certification preferred; must be willing to obtain if required
nMust be able to successfully complete all required background checks and employment requirements

nHave at least one year of professional caregiving experience
nAre dependable and consistently arrive to work on time
nHave a reliable mode of transportation
nAre comfortable working independently
nHave experience supporting adults who need assistance with daily living
nCan remain calm and professional during difficult or unexpected situations
nTake initiative and do not need constant supervision to complete routine caregiving duties
