Director of Hospital Engagement, RN

• Work with referral sources, discharge planners, clients, and families to support transitions from facility to home-based care
• Use knowledge of state homecare regulations to work with regional Aveanna Healthcare locations and support positive client outcomes
• Meet with case managers daily at assigned hospitals
• Check the status of new and existing patients and ensure patients and/or caregivers understand what to expect while on service
• Create and carry out sales and market development plans aligned with company goals and objectives
• Provide regular feedback to corporate management regarding sales activity and market development success
• Complete documentation for new patient referrals and coordinate with branch teams to obtain paperwork for starting service
• Respond to patient questions and concerns; assess needs, status, and tolerance issues; troubleshoot and make recommendations
• Coordinate with the Payer Relations team and provide updates throughout patient onboarding
• Interface with physicians, dietitians, and case managers to clarify and manage patients’ clinical needs
• Follow up on branch requests, including insurance issues, medical eligibility, and clinical information for authorizations
• Assist with special projects, including community patient needs, training requests, and hospital discharge processes

Requirements

• Active RN license in the state of Florida
• Must have a valid driver’s license and driving record that meet company standards
• Ability and willingness to travel locally

• As an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate.
• Recent NICU, PICU, or pediatric critical care nursing experience preferred
• Previous experience at a Children's Hospital strongly preferred
• Home Care or Private Duty Nursing experience a plus

Demonstrates expertise in home care nursing, including patient assessment, care coordination, and compliance with state regulations. Strong communication and organizational skills are essential for effective collaboration with healthcare teams and managing patient transitions.
