The Designated Manager (DM) is responsible for the overall management, quality, and regulatory compliance of Zane Care Inc.’s Minnesota 245D-licensed Home and Community-Based Services programs. As the DHS-recognized Designated Manager, this position oversees daily program operations, supports person-centered service delivery, and ensures compliance with Minnesota Statutes Chapters 245D and 245A and other applicable requirements.

The DM works closely with executive leadership, Designated Coordinators, Program Supervisors, Direct Support Professionals, individuals served, families, case managers, and other team members. This role supports regulatory readiness, staff performance, program quality, operational consistency, and organizational growth.

Key ResponsibilitiesRegulatory Compliance and Program Oversight

The Designated Manager (DM) is responsible for the overall management, quality, and regulatory compliance of Zane Care Inc.’s Minnesota 245D-licensed Home and Community-Based Services programs. As the DHS-recognized Designated Manager, this position oversees daily program operations, supports person-centered service delivery, and ensures compliance with Minnesota Statutes Chapters 245D and 245A and other applicable requirements.

The DM works closely with executive leadership, Designated Coordinators, Program Supervisors, Direct Support Professionals, individuals served, families, case managers, and other team members. This role supports regulatory readiness, staff performance, program quality, operational consistency, and organizational growth.

Key ResponsibilitiesRegulatory Compliance and Program Oversight

  • Serve as the Designated Manager under Minnesota Statute 245D.081 and provide daily oversight of program operations.
  • Maintain compliance with 245D, 245A, applicable Minnesota rules, DHS requirements, and organizational policies.
  • Oversee licensing renewals, variances, audits, investigations, corrective action plans, staff training, service delivery, documentation, and incident reporting.
  • Conduct routine site visits and internal audits to verify compliance, safety, documentation, and quality standards.
  • Develop and implement policies, procedures, forms, and improvement plans as needed.
  • Ensure services and documentation align with each individual’s Coordinated Support Plan (CSP), Coordinated Services and Support Plan (CSSP), Individual Service Plan (ISP), Self-Management Assessment (SMA), Individual Abuse Prevention Plan (IAPP), person-centered planning tools, and identified supports and outcomes.
  • Review required records, including medication error reports, seizure reports, medication records, health and daily progress notes, individual outcomes, individual finances, communication books, and other program documentation.
  • Attend team meetings and appointments as needed, either onsite or in the community.

Program Organization and Communication

  • Ensure program activities are completed according to schedules, service plans, policies, and procedures.
  • Follow up with staff when required activities or documentation are incomplete.
  • Develop and maintain records that document program activities, daily progress, and outcomes for each individual.
  • Communicate effectively with owners, managers, supervisors, staff, individuals served, guardians, family members, case managers, day programs, medical professionals, and other team members.
  • Coordinate services across programs to improve consistency, effectiveness, and efficiency.

Quality Assurance and Reporting

  • Develop and oversee quality‑assurance processes for service delivery, documentation, outcomes, incidents, and program performance.
  • Submit required reports to DHS and other oversight agencies accurately and on time.
  • Review data to identify concerns, risks, patterns, and opportunities for improvement.
  • Report significant findings and recommended actions to leadership.
  • Prepare quarterly, annual, financial, and team reports as required.

Staff Supervision and Development

  • Work with the HR Director and leadership to determine staffing needs and address program requests.
  • Implement organizational human resources policies and follow up when expectations are not met.
  • Supervise Program Supervisors and Direct Support Professionals by providing direction, coaching, feedback, and accountability.
  • Ensure staff receive required orientation, training, and ongoing support.
  • Lead onsite supervision and quarterly staff meetings.
  • Complete or oversee annual performance evaluations for program staff.
  • Support staff development in person-centered practices, positive behavior supports, therapeutic interventions, developmental disabilities, mental health, vulnerable‑adult protections, and required reporting.
  • Assist with staffing coverage, crisis response, and program stabilization when needed.

Financial and Operational Management

  • Ensure programs operate within approved budgets.
  • Review program expenses and address concerns involving overuse or underuse of funds.
  • Maintain accurate and current business and individual financial records.
  • Monitor calendars, site coverage, appointments, meetings, deadlines, and required follow‑up.

QualificationsEducation and Experience

  • Associate degree in a related field required; bachelor’s degree preferred.
  • Three to five years of related experience, including at least three years in direct care.
  • Supervisory or management experience in a residential or HCBS setting strongly preferred.
  • Previous experience supporting individuals with disabilities, mental health needs, or other program‑related needs.
  • Knowledge of Minnesota 245D services, program management, person‑centered practices, and residential operations.

Licenses and Certifications

  • Valid driver’s license with an acceptable driving record.
  • Reliable vehicle and current proof of liability insurance.
  • Ability and willingness to transport individuals in a company or personal vehicle when needed.
  • Must follow all traffic laws and report moving violations that may affect driving eligibility.
  • Ability to pass required Bureau of Criminal Apprehension and Department of Human Services background checks.
  • Current First Aid, CPR, and AED certification, or ability to obtain and maintain certification.
  • Medication Administration certification or required training.

Knowledge, Skills, and Abilities

  • Knowledge of Minnesota 245D requirements, the Vulnerable Adult Act, and mandated reporting standards.
  • Ability to respond calmly and quickly during emergencies or critical situations.
  • Strong leadership, organization, communication, documentation, problem‑solving, and time‑management skills.
  • Ability to manage multiple priorities, make sound decisions, and follow through on deadlines.
  • Ability to build professional relationships and work effectively with internal and external team members.
  • Ability to supervise staff, provide constructive feedback, resolve concerns, and maintain professional boundaries.
  • Ability to identify operational needs and develop practical solutions.
  • Proficiency with word processing, email, internet systems, databases, spreadsheets, and basic accounting tasks.

Working Conditions

  • This is an in‑person position and is not remote.
  • Work will be completed in the office and at assigned residential programs.
  • Time should be distributed among assigned programs based on operational needs.
  • Schedule flexibility is required for open shifts, urgent staffing needs, crisis situations, meetings, appointments, and training.
  • Approved overtime may be required when necessary.
  • Calls, texts, and emails may occur outside scheduled hours. Routine matters do not require an immediate response when off duty, but an appropriate backup plan must be in place. Urgent crises or site concerns may require assistance.
  • Designated Managers share agency on‑call responsibilities. On‑call duties may include carrying the on‑call phone for one week, responding promptly, troubleshooting concerns, visiting programs outside scheduled hours, assisting onsite when needed, and visiting three assigned sites on a Saturday and/or Sunday during the on‑call week.
  • Designated Managers must maintain and share an updated work calendar showing availability, site visits, meetings, appointments, approved time off, and designated backup coverage.
  • Time away during the scheduled workweek must be requested and approved in advance by both owners whenever possible.
  • Participation in company events outside scheduled hours is voluntary. Some events may be paid depending on the event.
