Denver Colorado Post Acute Care Nurse Practitioner Position-Flexible Scheduling

A growing, physician-led medical group dedicated to providing high-quality care in Skilled Nursing, Long-Term Care, Assisted Living, Independent Living, and Rehabilitation settings is seeking a compassionate, motivated Nurse Practitioner  to join its Denver-area team.

Responsibilities

n
• Provide medical care and follow-up for patients in post-acute care settings.
n
• Collaborate with physicians, therapists, case managers, and facility staff to coordinate patient care.
n
• Complete timely and accurate clinical documentation.
n
• Participate in quality improvement initiatives and ensure compliance with regulatory requirements.
n
• Travel to assigned Skilled Nursing, Assisted Living, and Independent Living facilities throughout the Denver metro area.

Qualifications

n
• Family, Acute Care, or Adult Gerontology Nurse Practitioner Certified
n
• Active Colorado NP license, or ability to obtain.
n
• Post-acute care, geriatrics, family medicine, internal medicine, or skilled nursing experience preferred but will consider new graduate NP's.
n
