Primary Responsibilities
- Work in decentralized areas alongside other health professionals to promote safe medication use. Areas of decentralization include Medical Units, Cardiac Unit, Step‑Down ICU, ICU, Heart Failure Clinic, Anticoagulation Clinic, and Surgical Satellite.
- Monitor medication therapy regimens for contraindications, drug‑drug interactions, allergies, appropriateness of drug and dose for diagnosis, and patient‑specific dosing parameters.
- Provide information to professional staff, patients, and families to encourage safe, appropriate, cost‑effective drug regimens and patient compliance.
- Provide clinical pharmacokinetics, TP/N, and anticoagulation dosing services for all appropriate patients.
- Document clinical notes and interventions in the electronic medical record to ensure effective and appropriate communication with the healthcare team.
- Respond to codes to provide drug information and medication preparation services as needed.
- Provide in‑services to medical, pharmacy, and nursing staff.
- Serve as pharmacy extern preceptor and share clinical knowledge with pharmacy externs.
- Assist in department activities to meet regulatory agency standards and to improve patient safety and efficiency.
- Participate in hospital committees as assigned.
Qualifications
- Education & Experience: Bachelor’s or Doctorate in Pharmacy (PharmD) from an accredited school. Hospital pharmacy residency or equivalent experience preferred.
- Licensure: Licensure or eligibility for licensure in the State of Indiana.
- Knowledge & Skills: Strong interpersonal and communication skills, knowledge of pharmacy laws, antibiotic stewardship, anticoagulation monitoring, parenteral nutrition, and the ability to manage multiple priorities in a fast‑paced environment.
Work Environment & Physical Demands
- Ability to work efficiently under pressure.
- Extended periods of computer screen time.
- Some lifting and bending are required.
