Clinical Pharmacist

Job Summary and Qualifications

The Clinical Pharmacist is responsible for providing general clinical and pharmacy services in compliance with regulatory and hospital policies/procedures. The pharmacist will provide patient care through the use of knowledge, skills, and abilities in compounding and dispensing pharmaceuticals.

Responsibilities

  • Interpret orders and transcribe/verify in the pharmacy computer system efficiently.
  • Accurately compound and dispense pharmaceuticals.
  • Monitor drug therapy regimens for contraindications, drug‑drug interactions, drug‑food interactions, allergies, and appropriateness of drug and dose.
  • Consult with physicians and nursing staff regarding availability, dosage form, side effects, and potential incompatibilities of new and current drugs, and suggest cost‑effective therapeutic alternatives.
  • Maintain the hospital formulary, minimize nonformulary procurements, utilize therapeutic interchanges, and promote rational drug therapy selection to limit unnecessary procurement costs.
  • Provide accurate and timely medication information to healthcare staff and patients.
  • Maintain appropriate controlled substance records.
  • Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs defined by Pharmacy Leadership.

Qualifications

  • Graduate of a ACPE‑accredited School of Pharmacy; PharmD preferred.
  • Active pharmacist license in the state of practice, in accordance with law and regulation.
  • Minimum of 2 years of hospital pharmacy experience preferred.
  • Prior hospital pharmacy residency preferred.
  • Current pharmacist licensure in the State of Georgia required.
