Case Manager - Manager (RN / MSW)

In this leadership role, you’ll ensure the coordination of high-quality, cost-effective patient care through efficient case review, resource management, discharge planning, and communication with third-party payers.

Key Responsibilities
- Supervise and mentor case managers, RN case workers, and social workers
- Review patient charts and ensure proper documentation and levels of care
- Coordinate communication between departments and third-party payers
- Monitor and improve care transition processes and discharge planning
- Lead performance improvement projects and track key departmental metrics
- Manage department budget, operations, and staffing
- Develop and implement case management policies and procedures
- Ensure compliance with regulatory and accreditation standards
- Serve as second-level reviewer for discrepancies in level of care decisions
- Maintain current knowledge of healthcare trends, policies, and case management best practices
Qualifications
Education (Required):
- Graduate of an accredited nursing school or master’s degree in social work (MSW) or related field
Education (Preferred):
- BSN or MSW
- Certification in Case Management (e.g., ACM, CCM, CPHQ)
Licensure/Certifications:
- RN License (Oregon) OR LCSW/LMSW (Oregon)
- Case Management/CPHQ certification preferred (required within 2 years of hire)
Experience:
- Minimum 2 years in case management in a hospital/healthcare setting
- Prior supervisory or management experience
- Familiarity with insurance regulations, discharge planning, and clinical documentation
