This full-time opportunity will work 80 hours bi-weekly from 8am - 4:30pm. Additional pay for shift differential is available.

Summary:
This position is responsible for the evaluation and disposition planning needs of patients admitted to the Behavioral Health Unit (BHU) and will:
- Complete psychosocial assessments with a focus on patients' requirements as they transition to the next level of care
- Assess, plan, and coordinate patients' post-acute health care services
- Participate in interdisciplinary treatment rounds and other patient care conferences
- Share responsibility of conducting involuntary admission hearings in the role of hospital presenter
- Collaborate with psychiatric providers, nurses, other social workers/therapists, to foster a coordinated approach to discharge planning
- Accurately complete required documentations in a timely manner
- Coordinate with patients' outpatient care providers for needed services and resources as needed
- Communicate with utilization review staff on any denials, issues, or barriers to discharges
- Obtain needed authorization for needed outpatient services
- Connect patients to community providers including psychiatrists and therapists to ensure patients have access to prescribed medications
- Assist patients to schedule medical appointments including transportation resources
Qualifications:
- Empathetic toward individuals with complex health needs and their families
- Strong communication, writing, and customer service skills. Knowledge of community resources
- Ability to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
- Excellent time management skills and ability to problem solve and exercise good judgment
Education:
Master of Social Work or related field (REQUIRED)
Licensure and Certification:
LCSW-C (PREFERRED)
LMSW, LGPC (REQUIRED)
BLS/CPR Certification required
