Ambulatory Care Pharmacist: Disease Management Expert

Trinity Health MI is seeking an ambulatory clinical pharmacist to provide disease management, comprehensive medication review, and Population Health Management across multiple sites under a collaborative practice agreement.

The role emphasizes collaboration with medical staff, care managers, and other providers to implement disease‑state programs, promote safe, evidence‑based, cost‑effective therapy, and educate staff and patients. Michigan license and experience required; BCACP preferred.
