Afterhours Hospice Care Manager RN

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. You will assess and coordinate patient and family needs during after-hours shifts, maintain clinical oversight, and serve on the Interdisciplinary Group (IDG) to ensure coordination with physicians and the hospice team.

Responsibilities include direct patient care, care plan updates, chart reviews, and regulatory compliance.
