Position Summary

In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our Chronic Care Management, Remote Patient Monitoring, and Behavioral Health services.

Key Responsibilities

• Establish the Tapestry Health patient relationship for our Chronic Care Management, Remote Patient Monitoring, and Behavioral Health services.
• Be available at a minimum of 10 weekdays per month to conduct approximately 300‑400 non‑clinical judgment consults onsite and remotely.
• These consults are brief, non‑clinical judgment consults, simply advising patients of the services they will receive with Tapestry Health. No prescribing or treating is involved.
• Document patient encounters accurately and thoroughly within our platform at the time of visit.
• Provide an end‑of‑day report to program administrators to track progress for completion of visits.

Minimum Requirements

• Qualified MD/DO.
• Successful completion of U.S. residency in Internal or Family Medicine.
• Active, unrestricted medical licenses in at least five states, including Minnesota.
• Excellent clinical judgment and decision‑making skills.
• Reliable transportation to travel to assigned buildings.
• Commitment to providing patient‑centered care and improving health outcomes.
• Familiarity with electronic health record (EHR) systems and healthcare technology.
• Ability to travel within 1–2 hours of the assigned region.

Compensation Structure

10–25k/month, mileage reimbursement, and malpractice coverage.
