Position Summary
In this contractor role, the MD will establish the Tapestry Health patient relationship for our Chronic Care Management, Remote Patient Monitoring, and Behavioral Health services.
Key Responsibilities
- Establish Tapestry Health patient relationships for Chronic Care Management, Remote Patient Monitoring, and Behavioral Health services.
- Be available a minimum of 10 weekdays per month to conduct approximately 300‑400 non-clinical judgment consults onsite and remotely.
- Provide brief, non-clinical judgment consults advising patients on the services they will receive; no prescribing or treating is involved.
- Document patient encounters accurately and thoroughly within our platform at time of visit.
- Submit end‑of‑day reports to program administrators to track progress and completion of visits.
Minimum Requirements
- Qualified MD/DO with successful completion of US residency in Internal or Family Medicine.
- Active, unrestricted medical licenses in at least five states.
- Excellent clinical judgment and decision‑making skills.
- Reliable transportation and ability to travel within 1‑2 hours of the assigned region.
- Commitment to patient‑centered care and improving health outcomes.
- Familiarity with EHR systems and healthcare technology.
Compensation Structure
10‑25k/month, mileage reimbursement, malpractice coverage.
