Wraparound Coordinator
The Wraparound Coordinator provides services to families with children and young adults up to the age of 21 who are involved with other community agencies such as Probate Court, DHS and Public Health; and who are at-risk for out of home placement. Responsible for the successful formation and functioning of the Community and Family Teams. Coordinate and follow the delivery of all services between agencies and the family. Services will be provided in the Family home, school other community settings. Persons with Lived Experience with mental illness, intellectual/developmental disabilities or substance abuse disorders are encouraged to apply.
