Job Description
- Engage families to better understand student needs and coordinate connection to
critical District and community services. - Coordinate with the school, clinic, and threats teams to facilitate transition of
services, assist clients with system navigation, and facilitate external referrals to
community providers for clients who are outside of the department's scope of
practice. - Case manage students and families who have recently denied or discharged from
mental health services. - Support new referrals by providing parent education regarding available services
and related paperwork. - Provide outreach and parent groups for assigned cluster on available Mental Health Services and related topics.
- Identify, support, and maintain provider database and liaison with these providers to facilitate student referrals.
- Liaison with clinical staff and attend on-going and relevant cross-discipline training.
- Effectively, efficiently, collaboratively, and cooperatively perform all other tasks and duties as assigned.
Qualifications
- High School Diploma or GED required.
- Preferred Social Work or related field.
- Three to five years' work experience with children who have experienced trauma required.
- Knowledge of community providers that address critical issues such as mental health, food
insecurity, or other challenges that can impact a student's readiness to learn. - Individual should be resourceful, detail-oriented, and flexible, possessing a positive attitude, cultural sensitivity, and ability to maintain accuracy and confidentiality in all work.
