Position Overview

Wound Care RN responsible for daily management and delivery of wound care services within the homecare setting, ensuring outcome‑based, cost‑effective care, and maintaining clinical quality standards.

Responsibilities

  • Participate in daily management and delivery of wound care for the Wound program.
  • Ensure outcome‑based, cost‑effective wound care using metric‑driven decisions.
  • Collaborate with wound team members and other clinical staff to achieve exceptional outcomes.
  • Perform wound care, oversee clinical staff, and maintain clinical care standards for clients receiving specialized wound care treatments.
  • Develop, direct, and manage daily operations and delivery of wound care, and contribute to program goals and objectives.
  • Manage a wound care caseload and LPN staff, ensuring consistent delivery of quality care and effective communication.
  • Seek consultation with WOCN staff to identify and plan interventions for complex client care problems.
  • Develop an interdisciplinary approach to client care with Wound program/Agency staff.
  • Direct and coordinate all client care services, including necessary medical/surgical services, to achieve patient outcomes.
  • Inform physicians and wound clinics of changes in client needs and plan of care for wound treatments.
  • Determine staffing needs and coordinate patient scheduling to meet service demands.
  • Accountable for team performance in achieving desired clinical and operational performance measures.
  • Follow trends in the specialty area of wounds for home care and recommend treatment options for program incorporation.
  • Participate in training, coaching, onboarding, and professional development of the wound program staff.

Other Duties

  • Actively participate in the performance improvement process known as H3W.
  • Participate in clinical education to provide programs aimed at improving clinical competencies of direct service staff.
  • Research new information related to wound specialty and communicate it to the team.
  • Perform other duties as assigned.

Qualifications

Education: Associates degree or diploma in nursing; Wound/Ostomy specialty certification and Bachelor's degree in nursing preferred.

Experience: Minimum of two years nursing experience in a clinical specialty area required; minimum two years in homecare preferred.

Licensure, Certification, Registration: RN License required; obtain Wound/Ostomy certification within 6 months of hire.

Knowledge, Skills, and Ability Requirements

  • Effectively communicate at all levels within the organization and share knowledge, ideas, and information.
  • Demonstrated success in management, planning, and leadership ability for meeting patient outcomes.
  • Ability to understand problem situations, solve problems, and independently assess a wide variety of tasks to identify solutions benefiting the client and agency.
  • Knowledge of relevant industry standards for wound care treatments.
  • Ability to balance financial, quality, people, and customer expectations appropriate to business situations.
  • Intermediate to advanced proficiency in MS Outlook, Word, Excel, PowerPoint, and Microsoft Project.
