We are seeking a NursePractitioner or Physician Assistant to provide high-quality, patient-centeredcare to residents in skilled nursing facilities throughout Mt. Pleasant, MI.This role offers clinical autonomy, flexible scheduling, and the opportunity tomake a meaningful impact in post-acute and wound care.

Key Responsibilities

  • Conduct on-site rounds andcomprehensive patient assessments.
  • Diagnose and manage acuteand chronic conditions, including wound care needs.
  • Prescribe medications anddevelop individualized treatment plans.
  • Address acute and emergentclinical issues.
  • Communicate effectively withfacility nursing staff via phone or secure messaging.
  • Maintain timely, accurateclinical documentation using a company-provided iPad.
  • Maintain reliable on-siteattendance and comply with clinical, facility, and regulatory standards.

Schedule & WorkEnvironment:

  • Part-time opportunity withflexible scheduling.
  • Facility shifts aretypically 6 hours, with rounds generally beginning between 7:00–10:00 a.m.
  • No on-call or overnightrequirements.
  • iPad with LTE provided formobile charting.

Qualifications:

  • Active, unrestricted U.S. NPor PA license.
  • Board certificationrequired: NP: ANCC, AANPCB, PNCB, orAACN. PA: NCCPA.
  • Clinical experience inpost-acute care, skilled nursing, wound care, or a related setting preferred.
  • Qualified backgroundsinclude Internal Medicine, Family Medicine, Emergency Medicine, GeneralPractice, Physical Medicine & Rehabilitation, and Geriatric Medicine.
  • Strong clinical judgment,communication skills, and commitment to patient-centered care.
