nOur team, CCF Group, is seeking a Wound Care Physician Assistant or Nurse Practitioner to support our expanding clinical services. This role includes rounding within long-term care facilities in Tulsa and/or Oklahoma City (OKC), with the flexibility to complete charting remotely. We are looking for a provider who shares our commitment to high-quality, patient-centered wound care. To support your work, the owners of CCF Group have developed a physician-designed wound care app that allows providers to monitor, document, and manage wounds with confidence, track healing through a mobile platform, and seamlessly integrate documentation into our EMR. We serve patients who rely on us for compassionate, comprehensive wound care, and we are seeking a licensed Wound Care PA/NP who can deliver skilled clinical assessment and treatment while upholding the standard of care that defines CCF Group.
nResponsibilities
nExamine patients and perform comprehensive wound assessments.
nObtain detailed medical histories and document clinically relevant findings.
nEvaluate, diagnose, and manage acute and chronic wounds, including pressure injuries, diabetic ulcers, and surgical wounds.
nDevelop and implement individualized wound care treatment plans in collaboration with physicians, nursing staff.
nPerform wound debridement and prescribe wound care therapies and medications in accordance with scope of practice and physician collaboration requirements.
nInterpret diagnostic tests and clinical data related to wound healing and infection management.
nEducate patients, caregivers, and facility staff on wound care plans, prevention strategies, and expected outcomes.
nMaintain accurate, timely, and compliant clinical documentation, including assessments, treatment plans, and follow-up notes.
nCoordinate care and case management responsibilities across long-term care, skilled nursing, assisted living, and home health settings.

nQualifications
nEducation:
nGraduate of an accredited Physician Assistant or Nurse Practitioner program
nActive, unrestricted PA or NP license in the State of Oklahoma
nNational certification (NCCPA for PA or appropriate NP board certification)
nDEA registration (or eligibility to obtain)
nCurrent BLS certification
nExperience:
nMinimum of 2 years of clinical experience, preferably in wound or primary care.
nExperience in long-term care, assisted living, or home health preferred
nSkills
nProficiency in wound assessment, treatment planning, and infection management
nStrong experience with EHR/EMR systems, clinical documentation, and charting
nKnowledge of wound care products, dressings, and debridement techniques
nAbility to collaborate effectively with physicians, nurses, and facility staff
nProficiency in English, both verbal and written
nPhysical & Environmental Requirements
nAbility to see with corrective eyewear and hear clearly with or without assistance
nMay be exposed to infectious or contagious diseases and work in close contact with patients
nRequired to wear personal protective equipment (PPE) as necessary
nPhysical Demands:
nOccasional bending, squatting, kneeling, twisting, lifting, carrying, pushing, and traveling
nAbility to assist with patient mobility as needed, including wheelchair transport
nAbility to lift and carry up to 15–20 pounds
